REQUIREMENTS FOR STUDENTS
SCHOLARSHIP TO BE MAKING YEAR 2012
Mandatory Requirements:
1.
Cumulative IP for scholarships PPA, PPE and TA at least 3.00
2.
IP Cumulative 2.50 minimum fuel for scholarships
3.
Minimal has taken 2 semesters
4.
Submit a copy of current KTM 1 sheet
5.
Submit a photo copy KHS until last semester each 1 sheet endorsed by Kajur
6.
Submit a copy of the Family Card (KK) 1 sheet
7.
Submit a photo copy of electric bill (bill last) 1 sheet
8.
Submit Personal Data / Curriculum Vitae (free-form and of the pr i badi).
9.
The certificate is signed by the chief work of Village / local community
leaders / parents
10.
Special scholarship fuel, MUST include Certificate of Ability of Head Village /
local community leaders.
11.
Parents income per month (U.S. $):
· Self
Employed / odd jobs signed by the Head of the Village / local community leaders
/ parents
· For civil
servants / Private continue to show a pay stub from the treasurer's salary
12.
Submit personal data / curikulum vitae student (format attached / available
faculty)
13.
Copy of bank account jatim
Additional Requirements:
1.
Certified as a participant level internal / External campus at least 2
or more with respect to the issuance of the latest year.

Late and incomplete requirements gathering, the file WILL NOT BE PROCESSED.
